This is a two day workshop based on the successful work of the 'Life Options' and 'Promoting Independence' projects, where Person Centred Planning has helped young people with learning disabilities to find a better way through this difficult life change.

Course Aims

This course looks at how we can better involve young disabled people in their transition from school to adulthood, using some tried and tested approaches.

Designed for

This course is aimed at staff who have an active and informative role in supporting young people with a learning disability through transition, including education, social care and health staff amongst others.

After the course participants should be able to:

  • Have a clear understanding of what is needed to make a good transition process
  • Have practical knowledge of how we can properly include young people and their families through the use of Person Centred Planning approaches
  • Share examples of a person centred approach to transition
